Yes — if Kayle is chosen in both executions, the deflect cost is paid twice: 3 once for the initial effect and 3 again for the repeated effect, 6 Power total (any Domain).

Each execution of Bellows Breath makes its own choices (rule 820.2.a), so the repeat execution choosing Kayle is a second "time they choose me," which is what rule 809.1.c charges for. Note the base cost is unaffected — she still takes 1 from each execution for 2 total, and the Deflect power can be any Domain.

Caveat: the deflect cost is a mandatory additional cost paid as you play the spell, so those targets and payments are locked in up front (rule 356.2.a.2). If the caster declines the Repeat cost, or repeats but chooses Kayle only in the first execution, Deflect 3 is paid just once. (One older FAQ entry claims Deflect is once-per-spell; the current Core Rules wording and the bulk of FAQ rulings go per choosing.)
